Family Centred Services (Family & Dometic Violence Support)

Our role is to provide direct support to whānau or individuals experiencing or affected by family or domestic violence. The primary aim of our service is to restore safety, foster wellbeing and promote positive change(s) to eliminate and prevent violence from reoccurring. See our brochure for full details. Whānau Ora Navigator Service

Kaiārahi (or Navigators) work closely with whānau to identify their specific needs and aspirations, helping to identify the services or support they require. The aim of this service is to enable whānau to grow and thrive by removing any barriers and building plans to take better control of their lives and circumstances. See our brochure for full details.

Service types: Whānau Ora, Kaupapa Māori, Service navigator.

Toiora - Rangatahi Support & Mentoring Service

Toiora is a youth support and mentoring service. Designed to support ‘at risk’ rangatahi to build essential life skills, make better informed decisions and cope with life challenges. Mobile Nurse & Healthy Lifestyle

Whanau Ora Chronic Disease Management Service: Our mobile nursing service targets Māori adults aged 25 years and over who have or are predisposed to a chronic condition including; diabetes, respiratory and, or heart condition. Our Mobile Nurse and Healthy Lifestyle Service is staffed by a Registered Clinical Nurse and Healthy Lifestyle Coordinator. Both are dedicated and committed to improving whānau wellbeing. Our team apply their clinical and community knowledge, skills and networks to ensure that you have all the necessary support to manage and improve your health. See our brochure for full details.

Service types: Community nursing, Family / whānau support, Wellness / healthy lifestyle, Exercise and activity, Healthy eating education, Mindfulness and mental wellbeing, Stop smoking.
