Public Service, Hospital-based support service, Hospital Pharmacy

Description

The Inpatient Hospital Pharmacy  supplies the medicines and provides pharmaceutical care to patients in hospital.

Clinical pharmacists visit the hospital wards on a regular basis to make sure medicines are being used safely and in a way that provides the best possible care for each of our patients.

The pharmacy team works very closely with the medical and nursing staff to identify and resolve any problems with medication, as well as finding ways to adjust medicines for the best results.

This involves speaking with patients and their family, whānau and caregivers. While patients often speak with a pharmacist soon after admission to hospital, individual patients can also speak with pharmacists during admission. This might happen because the medical team has asked, the pharmacist knows you need more information, or because you have asked to speak to a pharmacist. Understanding your medicines is important and we want to help you do that.

Please note: all patients are encouraged to bring their medicines into hospital with them. This is so we can check that we know exactly what medicines you should be taking.

Inpatient pharmacies are at both North Shore and Waitakere Hospitals.
